CANCELLATION POLICY
For Missed Appointments or Late Cancellations:
To avoid a missed appointment fee, please notify our office at least 24 hours in advance if you need to cancel or reschedule.
- After the first missed appointment or late cancellation (less than 24 hours' notice), a credit card will be required to reschedule future appointments.
- Any future missed appointments or late cancellations will incur a $50 non-refundable fee.
IMPORTANT PRACTICE INFORMATION
Mountain Vista Eyecare requires a retinal & macular image as standard of care at every annual eye exam. The fee for this imaging is $49.00.
If you need to cancel or reschedule your appointment, kindly give us a 24-hour notice. Appointments cancelled or rescheduled less than 24 hours in advance may be subject to a $50.00 fee.
